School Nurse

As school nurses, we are responsible for the health and safety of all Woodland students. Feel free to contact us with any questions or concerns that may affect your child’s experience at school.
The following must be submitted to the nurses office before the first day of class:
- Tennessee Certificate of Immunization**
- Copy of Birth Certificate**
- Health History Form
- Allergy/Food Allergy Action plan - (if applicable) signed by physician
- Emergency Medications - i.e. epinephrine injectors, rescue inhalers
- Medication Authorization Form - (if applicable)
**An up-to-date Tennessee Certificate of Immunization and Birth Certificate must be submitted prior to the first day of school. Any student without this documentation will be unable to attend class until documents are submitted and reviewed by the school.**
Health Forms
- Allergy Action Plan (non-food related) - must be completed and signed by student's physician
- Asthma Action Plan
- Food Allergy Action Plan - must be completed and signed by student's physician
- Food Allergy Questionnaire
- Health History Form (sent with the back-to-school online registration forms in the Family Access Portal each summer)
- Seizure Action Plan - must be completed and signed by student's physician
- Student Medication Authorization Form
- TSSAA Sports Physical Assessment Forms
